- ALL CLIENTS ARE ARE REQUIRED TO COMPLETE OUR COVID-19

PRECAUTION AND PROTECTION FORM ALONG WITH THEIR INTAKE FORM PRIOR TO THEIR 

APPOINTMENT.

- ALL CLIENTS MUST ARRIVE NO EARLIER THAN 10-MINUTES BEFORE

THEIR SCHEDULE APPOINTMENT.

- ALL CLIENTS MUST sanitize THEIR HANDS UPON ARRIVAL.

- CLIENTS AND GUESTS ARE REQUIRED TO WEAR A PROTECTIVE FACE

MASK AT ALL TIMES IN THE SHARED RECEPTIONIST AREA.

(FACE MASKS ARE OPTIONAL FOR CLIENTS WHEN RECEIVING SERVICE)

- ALL THERAPISTS AND PRACTITIONERS ARE REQUIRED TO WEAR A

PROTECTIVE HOSPITAL GRADE FACE MASK AT ALL TIMES FOR CLIENTs

SAFETY.

- ALL THERAPISTS AND PRACTITIONERS HAVE THE RIGHT TO REFUSE

AND TERMINATE SERVICES.

please notify your Therapist if you are not feeling well and we will kindly

reschedule your appointment. Refunds are not permitted at this time.

Memberships

HOW IT WORKS:
• Your monthly membership price will be automatically drafted from you debit or credit card each month,
entitling you to a single “Any 60-90-120 Minute Session” each month, on or after your scheduled
payment date.
• You may upgrade to a longer session at an additional charge.
• If you do not use your session for that month, it will simply roll-over into next month.
FREEZING MEMBERSHIP:
There may be times when a member will need to freeze his/her membership for a short period of
time, due to circumstantial reasons such as military leave, pregnancy, extended illness, relocation, etc. 
When a membership is frozen, the member is not authorized to use his/her membership services or benefits during the freeze period.

CANCELLATION OF MEMBERSHIP: 
After the initial 3 month term, you may cancel at any time. All cancellations require 30 days
written notice or email and are effective within 10 business days after the 30 day notice period. 
Payments due prior to the effective date will be charged as scheduled.

CANCELLING YOUR APPOINTMENT:
Cancellations without 24 hour notice may result in a 100% charge of your session, as that time
has been set aside specifically for your appointment.

NO SHOW APPOINTMENTS:
No shows are not tolerated and will be subject to the full charge of booked services.
No show appointments are non-transferable.


Reservations:

Appointments are required. At this time, we do not accept walk-ins at Manifest Station DC but we will do our best to accommodate same day requests. It is strongly encouraged to schedule at least a couple of weeks in advance to ensure availability.

Appointments can be booked online and a 50% deposit is required to secure your appointment to prevent no shows and last minute cancellations.

*Please notify your therapist of any allergies, sensitivity to smell or products, if you are pregnant or nursing, recent injuries or surgeries, or other medical conditions BEFORE your session.  

Cancellations:

We require a 24-hour cancellation notice.

No shows and last minute cancellations are charged the full amount of the service scheduled. 

Please be mindful that there are no refunds however clients can request to reschedule services with proper notification.

Late Arrivals:

We will do our best to accommodate late arrivals. However, we may need to reschedule your appointment or provide an abbreviated service in order to accommodate previously scheduled clients. You are responsible for full payment of service scheduled.

Gratuity:

Tipping is not required, but appreciated and at clients own discretion.

Gift Certificates: Less

Gift certificates are non-refundable and are not redeemable for cash. Manifest Station LLC is not responsible for lost, misplaced or stolen gift certificates.

Code Of Ethics: Less

Manifest Station LLC service providers will only work within our scope of practice. Our treatments do not take place of those of a medical doctor. If we are unable to assist you, we will refer out. If you have had a recent injury or surgery, or are currently under the care of a health care provider, it is your responsibility to get clearance from your doctor prior to booking your appointment.

Manifest Station LLC is operated by a licensed and insured professionally trained practitioner. Unprofessional or inappropriate behavior and conduct is strictly prohibited and will not be tolerated. If a service provider becomes uncomfortable, your session will be terminated immediately and you will be permanently banned from the establishment.  


Manifest Station LLC reserves the right to change policies, regulations, and pricing at any time upon providing a reasonable notice.
